The Racing and Wagering Board announces the release of newly revised charitable gaming forms.   The new forms have been updated to provide a more user-friendly format and clear instructions.   Electronic Forms listed below have been modified as of April 14, 2010Please discard all old versions of forms you may have in your possession and begin using the new forms immediately.

How to Print Forms: You must click on the print button that is displayed on the form. DO NOT select the print icon on the very top of your screen (that is part of your internet browser: Internet Explorer, Netscape, etc...).  If you get a blank page when you print the forms that means you are clicking the wrong print button.



CHARITABLE GAMING FORMS

General Forms for Games of Chance and Bingo:

Games of Chance:

*  Use these forms for Bell Jar, Las Vegas Nights, Carnivals and Bazaars.
*  GC-2, 2A and 2B must always be filed together.

Raffles with net-profits under $30,000 for the calendar year:

Notice: The New York State Racing and Wagering Board's Bureau of Licensing, in collaboration with the Division of Criminal Justice Services, is implementing electronic fingerprinting effective January 1, 2010.

Pursuant to the State contract between DCJS and L-1, the vendor fee can be altered twice annually dependant upon the amounts submitted under the State contract. The range of adjustment permissible is up to $11.75 or down to no lower than $9.95 per card.
